Please read our Registration & Policy Packet completely before filling out the Registration & Enrollment Form (you will find the Registration & Policy Packet on the Registration Packet Tab above.)

Please fill out and sign all the information requested.
(There are 3 pages of the Enrollment Form you need to fill out, sign & return to us.)
1- Davis Dance Academy Enrollment Form
2- Parent Financial Agreement
3- Release and Waiver of Liabilty and Indemnity Agreement

 Return the entire Registration Form to us at the studio with your $15.00 registration fee
& $20.00 recital fee. (recital fee includes 2 recitals~~January & June).

You will also need to pay June 2010 tution for one week.  (It is easier to collect this at the beginning of the year.) 

Recitals will be Wednesday January 27th & Wednesday June 9th
with Dress rehearsal being Tueday January 26th &  Tuesday June 8th
